Since I have strapped 24/7 for the past several years, I'm always jocked during doctor's appointments. My regular doc is a female and I could tell she was a little taken aback to see me in one but I think she has gotten used to it over the years. My urologist, male, doesn't seem to care either. A few years ago, I had to get an ultrasound for a kidney stone. The tech had me get partially undressed so he could do the scan. I pulled my jeans down, exposing my swimmer jock, pulled up my shirt and laid on my back on the table, He scanned my stomach, then had me lay on my sides, and then my stomach. He seemed to be really taking his time with those scans.

It would feel weird to me to wear a jockstrap to a doctors appointment, but then again I only wear them for athletics.
I do wear my regular white briefs whenever I go. But those are also my regular go to underwear
