Under the Apollo space suits I'm missing photos and context of the whole urine collection device but seems like over long johns ,or over liquid cooled underwear in the extravehicular suit, (with a hole presumably) they wore a specially sewn athletic supporter with a reinforced penis hole to which fitted a sheath collection tube. I wasn't finding mention of diapers in either the intravehicular or extravehicular suits so I dunno. Someone on Twitter did turn up a photo they are attributing as a used lunar jockstrap.
I haven't been able to find it recently but I solemnly swear I read an article that shared an anecdote from one of the astronauts recounting how they were checking supplies sometime after launch, radioed houston they couldn't find their athletic supporters. well eventually they opened the right panel or container and inzero/micro gravity their supporters just floated out into the cabin. Given the particular job they were tasked with that would have been awkward without them. If I ever find that again I'll post it.

McGlynn is the owner of the last jockstrap worn on the moon, an unusual artifact worth an estimated $10,000 and part of his vast collection of space memorabilia that includes spacesuits, flags, and flight logs of US and international space missions. McGlynn, 61, began collecting 55 years ago, when his brother’s school science project brought them to Raytheon Co., where they were given a piece of Mylar from the Echo satellite. The jockstrap’s original owner, Apollo 17 astronaut Eugene Cernan, is a friend of McGlynn and gave it to him as a gag gift. Advertisement “He told me, ‘Normally I wouldn’t do something like this,’ ” McGlynn recalled, “ ‘but you’re just juvenile enough to get this.’ ’’
Suiting begins with crewmen slipping into their long-john underwear up to their waists at which time the biomed technicians placed the biomed censors on the chest and a preliminary EKG test check is made. After buttoning up the upper part of the underwear, the athletic supporter and urine collection device was donned. I might point out that two sets of underwear were used. The cotton long-johns were used for the launch and landings while the liquid cool underwear was stored until needed for the moon landing. Also two types of space suits were used. Mike Collins wore an intravehicular suit, which means that these suits were only to be used inside the spacecraft. While Neil and Buzz wore extravehicular space suits.
