The only time it might have been practical for me to wear a jockstrap backwards was when I had surgery to remove a pilonidal cyst.

This resides nestled at the top of the buttcrack, and it's at the seam of that internal body cavity. The doctor disputes this but arguably that cyst is a vestigial scent gland hence the pore. Mine would swell up painfully and only rarely sometimes come unplugged in a mess with the application of manual pressure. It eventually developed a new channel to the surface in deep, open pimple, an infection risk uncomfortably close to the spine so it had to go. When the surgery is done it drains and drains, progressively doing less so over the course of maybe a month? Of yore it used to be that surgeons left the incision fairly open after surgery and you had to repack it with gauze over and over and over to sop away drainage. Nowadays usually and as was the case for me, they stitch it it shut and insert a drain tube to exit elsewhere in the butt cheek, that drains into to a little rubbery plastic bulb that maintains a little negative pressure as it is trying to return to full shape because it its squeezed flat when you reattach it after emptying it twice, once a day as needed. The incision itself was just a pinch oozy and/or moist, maybe all big incisions are but that's my only nondental surgery I have to reference besides stitches from cuts. Changing adhesive bandages sucks soooo much when they're reapplied over and over, and adhesive buildup gets especially painful to remove. So I switched later to using incontinence pads, mild adhesive to the underwear instead of to the skin. I neglected to photograph this, but I did try once or twice and see if I could keep either that pad or a nonadhesive gauze pad in place using a backwards jockstrap. Might have worked better with a different style pouch than I owned at the time, the athletic mesh distorted every which way. Anyways here's a super mild photo.
